The Central Intake Program is the single point of access to coordinate referrals to the right care, the right place, at the right time with integrated communication between all parties. Our team consists of health care professionals and administrative staff who work with health care providers, service providers and patients to access services easily and in a timely manner, supporting consistency and best practice among services. The Central Intake Program processes referrals for the below services: • Diabetes Education • Addictions and Mental Health • Musculoskeletal Care Rapid Access Clinics for: o Hip & Knee o Low Back Pain
